Marching Band Forms

The form requirements for marching band have been simplified and the number of required forms has been reduced.  Please go through all forms and return the Medical/Parent Consent Form and the Instrument Rental Agreement (if needed for your child.)

All band members and parents should be familiar with the MVHS Band Handbook

Most forms are now set up for you to type in your information on your computer, print and sign before sending them in with your band member.  If you cannot print the forms, hard copies are available in the MVHS Band Room.

Description

Medical / Permissions Form

All marching band students must have this form on file.  It will provide necessary emergency medical information and permission to seek treatment in case of emergency. 

Band Instrument Rental

The Stafford County Public Schools provide a number of large and specialty instruments for rental by students enrolled in the band program at MVHS.  SCPS does not generally provide flutes, clarinets, alto saxophones, trumpets or trombones.  This rental agreement must be completed before a student is issued a school-owned instrument.  Students using multiple instruments (marching + concert) only pay one instrument rental fee.

The Marching Band Field Trip Permission form must be completed and returned by all members of the group prior to our first performance date.

W-9

Students (parents) may "opt in" to allow profits from fund-raising to be reserved into a student account to be used toward the payment of band fees.  Guidelines and more information on the W-9 form and Student Accounts are available on the Student Accounts page.
