Marching Band members must have each of the following forms on file in the band office in order to participate in rehearsals or performances with the Mountain View Marching Band

Marching Band
Medical Release Form

All marching band students must have this form on file.  It will provide necessary emergency medical information and permission to seek treatment in case of emergency.  This form must be notarized, so please do NOT sign it without a Notary Public present.  An attempt will be made to have a Notary available at the mandatory parent meeting in August.  A new copy must be filed each year of participation.

Uniform Rental Agreement

A physical sign of our identity as a band that must be worn with pride and great care.  Each student will be required to complete the attached form which is to be signed by the student and parents to acknowledge responsibility for the care and proper wear of the Mountain View High School Band uniform.  Additional information on uniform expectations can be found in the Band Handbook.

Band Instrument Rental

The Stafford County Public Schools provide a number of instruments for rental by students enrolled in the band program at MVHS.  This rental agreement must be completed before a student is issued a school-owned instrument.  Students using multiple instruments (marching + concert) only pay one instrument rental fee.  Rental fees are due for students in all except the following marching band sections: Clarinet, Alto Saxophone.

Band Handbook Acknowledgement

All band students and parents will be required to return this form acknowledging receipt of the MVHS Band Handbook and agreement with the terms of enrollment and expectations for the MVHS Band program and each individual ensemble.

VHSL Physical Form

All members of the Mountain View Marching Band will be required to have a completed Virginia High School League Physical form on file prior to the beginning of August band camp. These forms are available from your school's athletic office or may be downloaded from the VHSL website. They are valid for one full school year for all athletic activities.

The Marching Band Field Trip Permission form must be completed and returned by all members of the group prior to our first performance date.

Internet Release Form Parents may wish to "opt in" to allow their student's individual achievements to be included on this web site.  This form may be completed, or and updated form may be completed, at any time.
